Services

Case management is designed to assist the individual and their support network to identify, select, obtain and coordinate services which are designed to assist persons with developmental disabilities. We encourage both paid and natural supports to increase independence and provide community inclusion and integration. Services further include, but are not limited to:

• Assessment:  Determine the individual’s preferred lifestyle and desired growth
• Support Planning:  Develop the plan to reach the individual’s goals
• Support Coordination:  Arrange and secure services to support the individual’s plan
• Advocacy:  Pursue access to needed services and supports
• Transition and Portability:  Assist in the planning and arranging of services when an individual’s lifestyle and goals change
